I have read the information regarding the costs for tuition, registration, accommodation and board for the period of study at the Institute. I certify that the above information is correct to the best of my knowledge. I am fully aware of the fact that the Enrollment Form will not be proceeded without giving all the required details and attaching the following documents: a copy of the identification page of the passport, a copy of the final secondary-school certificate (Program A), a letter of motivation to study Slovak (Programs B,C,D).
